These forms apply to both Independent and University Organizations

• Travel Addendum (for any organization travel)
  • Graduate and Professional organizations will not be reimbursed for any activities off campus. 
• Travel Waivers (for any organization travel)
  • Only University (IT) Organizations can use Cornell Fleet Services as a transportation resource. 

Independent (EO - External) Organizations

• Service Engagement Agreement (Performer, Entertainer, or Service Provider)
• Sports Official Form (just sports officials)

University (IT - Internal) Organizations

• Service for Performer with Equipment
• Service for Performer without Equipment
• Service for Graphic Design or Videography
• Service for Photographer, Artist, Illustrator
• Service for Language Translation and Interpretation
• Service for Writing, Editing, Indexing
• Speaker Reimbursement and Payment Form
• Service Provider Contract (all else)
